How does mobile PT work?

It's simple - Kansas City, we come to you! Home, daycare, school, ..... the world is our office! *Note that some insurance plans require services be provided in the home only. While it may be an option to meet at a public space such as a playground, please note that primary indoor location must be within the driving radius.

Do I need a doctor’s referral?

Access to physical therapy services is dependent on the state services are provided in. Services can be initiated in both Kansas and Missouri without a referral. In Missouri, your PT will need to consult with your referring provider prior to 10 visits/30 days from the start of treatment.

Although a prescription may not be legally required for treatment in your state, some insurance plans still require a prescription or signed plan of care from an approved provider. Medicare recipients must be under the care of a physician or an approved provider. 

What types of health insurance do you accept?

Function Forward is a participating provider with:

  • Blue Cross Blue Shield 

  • Medicare Part B for Missouri and Kansas residents

Function Forward is out-of-network with all other major medical plans.

Due to the nature of this small, intimate practice, there is not a separate billing department. To streamline admin and billing processes, a credit card is required to be on file for all new clients. 

How do out-of-network benefits work?

If we are not in-network with your insurance plan, you may be able to utilize out-of-network benefits to get reimbursed for physical therapy services. Verify with your plan, then we will provide you with the necessary documentation to submit for reimbursement. Some plans have very generous out-of-network reimbursement, so it is always worth it to check!

We accept credit/debit card (including HSA cards), checks, and cash.

There are no additional fees for home visits (unless you are outside the driving radius).

What is the procedure for getting started?

Reach out with a text or email to initiate contact. Then, we will schedule a brief call to ensure Function Forward PT is a good fit for your needs.  Next, you will be sent some intake paperwork to fill out electronically, so on the day of the initial evaluation we can get right to work! If you are using an insurance plan, a copy of your card is needed prior to the first session. 

What are the hours of operation?

Function Forward Physical Therapy clinical hours are Mon-Fri, 8:30-2:45pm. Please note hours may change due to therapist availability. We apologize, but late afternoon/evening hours are not available at this time. Function Forward Physical Therapy is closed on all major holidays. 

I've heard of pediatric and orthopedic physical therapists, but what exactly is a neurologic physical therapist ?

A neurologic physical therapist is a physical therapist who specializes in the evaluation and treatment of individuals with movement problems due to disease or injury of the nervous system.  Physical therapists can help improve or restore the mobility you need to move forward with your life. Many pediatric health conditions fall under the neurologic umbrella or relate to neuromotor development.

What is a Neurologic Clinical Specialist?

A Neurologic Clinical Specialist (NCS) is a physical therapist who has had advanced training or practice in the area of neurologic physical therapy. These PTs have been awarded the designation NCS because they have met the requirements and passed a rigorous specialty examination through the American Board of Physical Therapist Specialists. There are only a small handful of neurologic clinical specialists in Kansas City, most working a larger healthcare institutions. 

Do I need to be home-bound to receive mobile physical therapy services?

Absolutely not! Receiving services in the home is merely another option available to you. Mobile health services are NOT the same as home health services. For insurance, outpatient benefits are utilized.
For Medicare, home health is covered through Medicare Part A while  mobile services are covered through Medicare Part B.

What is the cost for services?

Function Forward is in-network with Medicare Part B and BCBS. Co-payments/co-insurance will be collected as indicated by your specific plan. For out-of-network and wellness services, the fee per 60 min session is $120.

Standard Notice: “Right to Receive a Good Faith Estimate of Expected Charges”

Under the No Surprises Act Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are NOT enrolled in a plan or coverage or a Federal health care program, or NOT seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

“Right to Receive a Good Faith Estimate of Expected Charges”  Under the No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

• Make sure to save a copy or picture of your Good Faith Estimate.
